TME · Tumor Microenvironment
一句話：腫瘤不是孤立的癌細胞，是癌細胞 + 周圍一堆非癌細胞（fibroblast、macrophage、T cell、血管、ECM matrix）共同組成的「生態系」。這個環境決定治療有沒有用。
主要成員：
- CAF（cancer-associated fibroblast）— stroma；分泌 collagen 讓藥打不進去；分泌 cytokine 阻擋免疫
- TAM（tumor-associated macrophage）— 兩極化：M1 反癌 / M2 促癌；大多數腫瘤裡是 M2
- Treg（regulatory T cell）— 壓制其他 T cell；免疫治療失敗常跟 Treg 過高有關
- MDSC（myeloid-derived suppressor cell）— 廣義免疫抑制
- ECM（extracellular matrix）— collagen / hyaluronan；物理屏障
- 血管（abnormal tumor vasculature）— 亂長 + 漏水 → drug delivery 難
