Codex synthesis script updated to align with case report format guidelines
04-synthesize-codex.sh 更新了委派給 codex 的風格守則，與 03-synthesize-prep.ts 的 template 改進同步。Codex 現在會收到醫學 case report 結構指示：narrative 50-100 字白話，學習錨點 4-7 條（涵蓋 7 種類型，優先 Best Practice 與 Guideline）。這確保整個 synthesis pipeline（prep → codex delegation → output）的格式一致性。Script 仍維持用 codex gpt-5.5 處理長文 narrative，避免消耗主 Claude session tokens。
